Legal
Privacy Policy
Effective [EFFECTIVE DATE]
This Privacy Policy explains how [LEGAL BUSINESS NAME] ("Lariat IT," "we," "us") handles information collected through lariatit.com (the "Site"). It covers the Site only. IT services we provide to client practices are governed by our written service agreements with those practices, including any Business Associate Agreements.
Information we collect
Information you send us. If you use our contact form, email us or call us, we receive what you give us. This is usually your name, email address, phone number, practice name, the topic of your inquiry and your message.
Technical information. Like most websites, our hosting provider automatically records basic technical data when you visit, such as your IP address, browser type, the pages requested and the date and time. We also use your IP address briefly to limit repeated contact-form submissions and prevent spam.
What we don't collect. The Site has no user accounts, takes no payments and doesn't use advertising or tracking cookies.
Please don't send patient information
The contact form and our general email address are not meant for protected health information (PHI). Please don't include patient names, dates of birth, chart details or any other patient information in the contact form or in a first email. If your request involves patient data, tell us only the general nature of the issue and we'll arrange a secure way to handle it under your practice's service agreement.
How we use information
We use the information described above to:
- respond to your inquiry and provide the information or services you request;
- schedule assessments, consultations or support;
- keep the Site secure, working properly and free of spam;
- comply with legal obligations.
We don't sell or rent your personal information, and we don't share it for cross-context behavioral advertising.
How we share information
We share information only:
- with service providers that help us run the Site and our business, such as our web hosting and email providers, who may use it only to provide those services to us;
- when required by law, or to protect the rights, safety or security of Lariat IT, our clients or others;
- as part of a merger, acquisition or sale of our business, in which case this policy continues to apply to the information transferred.
Cookies and third parties
The Site doesn't set tracking or advertising cookies. If we add analytics or other third-party tools in the future, we'll update this policy first.
How long we keep information
We keep contact-form messages and related emails for as long as needed to respond to you and maintain our business records, and then delete them. Server logs and spam-prevention records are kept only for a short period by our hosting provider and by us.
Security
We use reasonable safeguards to protect information, including encrypted (HTTPS) connections to the Site. No method of transmission or storage is completely secure, so we can't guarantee absolute security. This is one more reason not to send patient information through the Site.
Your choices and rights
You can ask us to access, correct or delete the personal information you've sent us through the Site by emailing contact@lariatit.com. Depending on where you live, including in California, Oregon and Washington, you may have additional rights under state privacy law. We'll respond to requests as the applicable law requires, and we won't discriminate against you for making one.
Children
The Site is intended for businesses and is not directed to children under 13. We don't knowingly collect information from children.
Changes to this policy
We may update this policy from time to time. We'll post the updated version on this page and change the effective date above.
